I authorize the Hong Kong Anti-Cancer Society to transfer the above specified amount from my credit card account every month. This authorization will remain in effect after the expiration of my credit card and after a new card is issued until further notice. We will make the first payment immediately, and subsequent monthly donations will be paid on the 15th of each month. 本人授權香港防癌會每月由本人的信用卡戶口轉賬上述指定金額。此授權在本人信用卡的有效期過後及獲發新卡後仍繼續生效，直至另行通知。我們會即時進行首次過賬，隨後的每月捐款則會在每月15號過賬。 *

The personal information you provide (including name, address, email address and telephone number, etc.) will only be used for the Association to contact you, including issuing donation receipts, member services and communications, fundraising, opinion collection, health seminars and related activities, and providing you with the latest cancer information. We will not provide your personal information to any third party for non-Association related purposes, nor will we sell or transfer your personal information to any third party.
