I authorize the Hong Kong Anti-Cancer Society to transfer the above specified amount from my credit card account every month. This authorization will remain in effect after the expiration of my credit card and after a new card is issued until further notice. We will make the first payment immediately, and subsequent monthly donations will be paid on the 15th of each month. 本人授權香港防癌會每月由本人的信用卡戶口轉賬上述指定金額。此授權在本人信用卡的有效期過後及獲發新卡後仍繼續生效，直至另行通知。我們會即時進行首次過賬，隨後的每月捐款則會在每月15號過賬。 *
